Client Rights, Records Requests, and Consumer Notices
This notice is posted in compliance with Texas House Bill 4224 (Effective September 1, 2025).
Records Requests
You have the right to request and receive a copy of your mental health records. To obtain your record submit a written request to your provider via email, secure portal, or mail. This is in accordance to Texas Health & Safety Code §611.0045. Your provider has 15 days to respond to your written request according to Texas Health & Safety Code §611.008(a).
Clients needing copies of their records, may submit a written request using one of the following methods:

To help prevent delays, please include the following information with your request: Client full legal name and date of birth, current phone or email address, description of records requested, preferred method of delivery (secure electronic delivery, mail, or in-person pickup).
If records are requested on behalf of another individual, documentation verifying legal authority is required. Examples include guardianship documentation or medical power of attorney.
Identity verification may be required to protect client privacy and confidentiality in accordance with applicable state and federal laws.
Any applicable fees for records can be found in your intake documents or requested at the same time as the records.
Complaints
If you ever have concerns, questions, or complaints regarding your counseling services, talk with your counselor first. Please provide as much detail as possible of the concern or complaint. Providing explicit feedback allows us to collaborate, adjust our therapeutic approach, and support your growth. If a resolution cannot be reached through open communication, you maintain the right to contact your counselor’s clinical supervisor, the Texas Behavioral Health Executive Council (BHEC), or the Texas Office of the Attorney General. Contacting the State Licensing Authority
The Texas Behavioral Health Executive Council (BHEC) is the licensing authority for Licensed Professional Counselors in the State of Texas. Questions regarding licensure or professional regulation may be submitted through the Council’s website: https://bhec.texas.gov/contact-us/

NOTICE TO CLIENTS
The Texas Behavioral Health Executive Council investigates and prosecutes professional misconduct committed by marriage and family therapists, professional counselors, psychologists, psychological associates, social workers, and licensed specialists in school psychology. Although not every complaint against or dispute with a licensee involves professional misconduct, the Executive Council will provide you with information about how to file a complaint.

Filing a Consumer Complaint
